Identify targeted geographic areas within each county to begin strategic healthcare practitioner outreach, using existing data sources that pinpoint high patient need areas.
Gather key stakeholders to fully identify challenges and opportunities
related to community-wide use of social needs referral platforms
Evaluate existing nutrition education resources that would benefit patients and healthcare practitioners in Central Florida.
Intentionally connect with healthcare practitioners understand their needs and challenges around screening for Food Insecurity, and gauge what materials and approaches would be most helpful.
