Report: Referral Platforms for Comprehensive Care Coordination

Technology based referral platforms have been expanding across the country, and here in Central Florida, over the past several years. The HHTF is committed to understanding the impact of these platforms in our community and aligning partners on use of these platforms through the joint Closed Loop Referral Workgroup. The CLR Workgroup was established in October 2024 as a shared workgroup across three Central Florida coalitions: the Health and Hunger Task Force, Ginsburg Institute for Health Equity Child Health Equity Workgroup, and Florida Department of Health Orange County Health Equity Task Force.

This new report provides an overview of the landscape of top referral platforms across the country, including examples of use cases, user experiences at the healthcare and community organization levels, and strategies for implementation.
